Step 1:
Locate the EyeMed provider most convenient to you. For providers near you, call toll-free 1-800-804-4384 or use the Provider Locator Service at www.ufugi.com. |
Step 2:
Schedule an appointment. When making the appointment, tell the office that you are EyeMed Vision Care Access Plan D member and provide your name and your social security number. |
Step 3:
When you arrive, identify yourself EyeMed Vision Care member and present your ID card. |
